• for the release of original study documents:

  • Application for the release of the diploma and transcript of records - AS1
  • Application for the release of the transcript of records or the duplicate of the transcript of records /Diploma supplement – AS2
  • Application for the release of the Syllabus - AS3
  • Application for the release of the student’s transcript of records – AS4
  • Application for the release of an authenticity certificate - AS5-a
  • Application for the release of a certificate - AS5-b
  • Application for the expedited release of the diploma and transcript of records - AS7
  • Application for the release of a new education document/Diploma supplement (if the holder finds errors in the original one) – AS10
     

• for the release of duplicates

Upon the release of the diploma, graduates are required to fill in the following questionnaire: Questionnaire of UPT graduate
